After contacting the Dietitian, a nutrition questionnaire, and short food log are emailed. Email or drop off completed forms, with any other relevant medical information. During the session, we talk about food habits, and your relationship with food, as well as health goals. A personalized nutrition plan is created with information from the forms, and session. A follow up session after 2-3 weeks is recommended to assess how plan is going, and make changes when needed.
