The Jamaica YMCA is seeking a Re-housing Specialist. As an employee of the YMCA, this position provides case management for transitional housing clients referred by HASA. Working with residents to secure safe and secure housing in a finite period of time, while working on relief of barriers to permanent housing.

Key Responsibilities: 

  • Upon completion of registration process, a member of the re-housing specialist team will the new client in registration. Acclimate new client to all amenities (meals, room keys, televisions, etc.), review program expectations, escort new resident to room, complete assessment to identify goals that will assist the residents with integration into transitional housing. 
  • Develop a rapport that lends itself to an open dialogue on the resident's barriers and complications that would delay or preclude the client's goals.
  • Develop a treatment plan that is realistic and doable in the time frame allowed of 180 days. Work toward re-housing the resident within the allotted time frame.
  • Support and refer to any services that would help the client meet said goals (substance abuse / mental treatment, housing appointment, etc.)
  • Access the client’s medical issues and keep track and document any changes. 
  • Provide a safe and secure residence where the can thrive and meet their goals.
  • Assist in the orientation of new personnel when requested. Act as a backup for other team members.
  • May have on call duties.
  • Perform other duties as assigned.
  • Other activities and duties that address the ongoing health and well-being of our staff and members.
  • Follow and maintain sanitary habits in accordance with CDC guidelines.

Desired Skills & Experience:

  • High School Diploma required.
  • Minimum of one (1) to three (3) years of experience. 
  • Knowledge of Case management, Substance Abuse Treatment, Mental Health, HIV Pre and Post, Cobra Case Management, working the chronically homeless and under housed.
  • Flexible work schedule/hours, preferably bilingual (Spanish).
