SALARY: $17.78 - $22.00 P/H

The Chinatown YMCA is seeking a Case Planner responsible to provide direct counseling for multi-problem minority families. Knowledge of community resources for youth and families, NYS and NYC child welfare system and bi-lingual abilities preferred, Chinese and or Spanish.

Key Responsibilities:

• Participate in the outreach and case finding activities involving Beacon Programs and ACS.
• Provide direct preventive services including individual, group and family counseling to assigned caseload of a minimum of 12+ families.  Evaluate psychosocial needs through regular home visits and other appointment.
• Conduct intake assessment of cases assigned by supervisor in order to obtain, analyze, and evaluate information related to client’s psychosocial problems and functioning.
• Provide case planning services.  This includes making referrals and doing follow-ups with the community and sponsoring agencies; to liaison and collaborates with agencies that clients’ are involved with; and advocate as needed.
• Provide crisis intervention as necessary when client/family is in an emergency situation.  Secure concrete support services to each family.
• Maintain accurate and up-to-date uniform case record, progress notes, and other records for all clients in accordance with Deliver high quality youth and family counseling program under the guidelines of the Preventive Foster Care ACS (Administration for Children Services) and/or specific funding source. 
• Create a positive relationship with ACS staff, OCFS staff, school guidance counselors, teachers, and fellow social work agencies.                  
• Provide positive and engaging casework practice that meets the ACS, NYS, OCFS standards resulting in positive program outcomes and positive audits/reviews.  Specific case work practice will result in increased, engaged families.    
• Other activities and duties as needed that address the ongoing health and well-being of our staff and members
• Maintain sanitary habits in accordance with CDC guidelines

Desired Skills & Experience:

• Bachelor’s degree in Counseling, psychology and sociology.
• Minimum of One (1) to two (2) years of experience.
• Bilingual Chinese/English; English/Spanish.
