The Chinatown YMCA is seeking a Community Pathway Worker, who will play a vital part in providing direct support to families within the school community. They will utilize motivational interviewing techniques to empower families to reach their goals and create nurturing environments for their children.

School-Based Early Support: Catchment 1, 2, 3 (Districts 1 & 2).
School Communities: P.S. 140, P.S. 142, P.S. 110, M.S. 131 & P.S. 42.

Key Responsibilities:

Family support and empowerment:

  • Provide case management services to (8) eight families who self-enroll in the program; utilize motivational interviewing techniques to support families in identifying their strengths, setting goals, and overcoming challenges; partner with families to develop and implement individualized support plans; connect families with appropriate resources within the community.
     

Parent education and support:

  • Co-facilitate evidence-based parent support groups alongside the School Outreach Liaison using the Triple P model, empowering parents with positive techniques to manage challenging behaviors and build strong relationships with their children.

  • Facilitate open communication and peer support within the parent groups and parents individually when needed.
     

Collaboration and outreach:

  • Work collaboratively with the School Outreach Liaison to support school partnership and outreach efforts; support the school environment and be able to meet the cases on biweekly basis, meeting their culture and language.

  • Assist with program events and initiatives focused on supporting families within the school community, meet with the families according to the standards and ACS requirements.
     

Develop and execute school-based offerings: 

  • Identify the needs of the school community and create engaging programs such as workshops (parenting skills, financial literacy), cultural events celebrating diversity, and youth events promoting social and emotional learning in collaboration with respective schools, and existing Chinatown YMCA programs currently in operation.
     

Build a strong and supportive school community:

  • Become a trusted liaison between school staff, families, internal Chinatown YMCA programs, and community partners.

  • Proactively build trust and rapport with families, building strong relationships with families from diverse backgrounds, promoting inclusivity and accessibility; meet with school teams to identify at-risk families; educate school staff about the program and its target population; integrate the program with school prevention plans as appropriate.

  • Keep the documentation updated on a biweekly basis and be able to serve the families according to ACS requirements and policies, keep all stakeholders informed through regular communication, including newsletters, social media, and meetings, track program participation, outcomes, and data in Client Track (agency database) to maintain accurate records and generate reports; implement data-driven improvements and stay up-to-date on best practices in family support and children's development.
     

Stay current on family support best practices:

  • Attend relevant trainings and workshops to continuously improve program effectiveness. Including but not limited to Motivational Interviewing, Trauma-informed care, cultural competency, and other workforce development trainings.


Desired Skills & Experience:

  • Bachelor's degree in counseling, psychology, or sociology.

  • One (1) to two (2) years of related experience.

  • Bilingual: Spanish/English; Chinese/English.
