The Vanderbilt YMCA is seeking a Case Manager who will work with the Learn and Earn-WIOA participants at the Julia Richman Education Complex. They will be responsible for increasing student achievement for college and career readiness. The Case Manager will provide students with a positive mentor relationship of advocacy and support to help them achieve their academic goals. They will work one-on-one with students around the concrete needs of youth and lead and run groups. Additionally, they will assist in designing and leading training sessions on behavior issues with youth and teens in the program.
Key Responsibilities:
Conduct intake and assessments of potential participants and develop individual service plans to ensure the success and implementation of case management. Manage case notes for program participants.
Facilitate groups and workshops based on the participants' expressed needs and interests to improve attitudes, school performance, and home and community relationships.
Provide case and behavior management and refer to outside resources and agencies as needed.
Manage case notes for post-graduate participants, also known as WIOA Follow-Ups.
Provide outreach resources for post-graduate participants.
Facilitate/Co-facilitate and write SEL and Teen Core Activities during program hours.
Enter case notes in the DYCD Connect database regularly.
Enter Teen Data in YMCA Youth Services and AS400 databases.
Recruit participants and collect documentation from them, schools, parents, and community partnerships.
Support the Coordinator in ensuring proper documentation is maintained and entered into the DYCD Connect system.
Prepare students for internship placements and provide support for internship site placements, including pre-and post-audits.
Improve, develop, and enhance all program service delivery.
Maintain and develop positive relationships with families, participants, staff, schools, volunteers, and other organizations.
Attend staff meetings, training sessions, and any applicable special events.
Desired Skills & Experience:
High School Diploma required. Associate’s degree or relevant experience desired.
Minimum of Two (2) years of experience working with teens and doing case management.
Experience teaching and facilitating training sessions on issues related to youth and teens.
Must be CPR/First Aid certified and complete the YMCA Mandatory Reporting Training within Four (4) months of employment.
